I just reinstalled Windows and made a registry change to delete the app shortcuts. It functions properly now. After each restart, they revert back to black squares.

Occasionally it does. It’s not a perfect reason, but it seems the system just struggles with input... Windows might be dyslexic sometimes. /shrug

Can you try the following? 1. Open Command Prompt as an administrator 2. Type regsvr32 /u Shell32.dll 3. Type regsvr32 Shell32.dll tell me what happens if you get a warning in any of the steps.
