Antivirus solutions might detect around three out of five real infections on a computer. Avast performs well in this area. Many tools—such as Adwcleaner, Malwarebytes, Hitman, Combofix, JRT, Rkill—can identify and eliminate other issues that others miss. For the typical malware I encounter (common among general users), it usually involves browser threats, fake antivirus apps, and speed-up scams. More challenging infections, like those from DoD/FBI ransomware, demand booting to a disconnected system to uncover fully compromised files. I can confidently say most infections stem from downloading updates or Java/JavaScript files and clicking the first link or opening an infected attachment. You can't be certain about every file, but you're likely safe with almost everything. The only sure way to completely remove everything is a full reinstall.
