Is the ISP combo operating in bridge mode? If not, you'd need to create a DMZ between it and the Apple router so that certain ports can go through the double-NAT setup. Alternatively, if it doesn’t support DMZ, you’ll have to forward each port individually from the ISP router to the Apple one.

If it’s suggesting bridge mode, then your Apple device should receive your public IP without needing any forwarding. This matches the setup where only one port connects to the Internet. Also, double-checking—does the Apple display a public WAN address? It’s also possible your ISP uses CG-NAT and isn’t providing a public IP at all.
