Ports for PS/2 connections aren't automatically enabled; your computer needs to be restarted for them to activate.
PS/2 equipment that isn't compatible with USB won't function with a USB PS/2 adapter.
Can you try testing the mouse on another computer, including without the adapter? Adapters can be unpredictable sometimes—make sure to check if they work. Also: in the second photo, the clear plastic tip looks pitted. I’m not sure about the bluish stains. Where was the mouse kept?
That is a passive adapter, which will only work with those rare mice that are PS/2 but also understand USB (it was much more common for USB mice to also understand PS/2, but even those went away 15 years ago).
In order for that mouse to work on USB you need an active adapter to translate the signals.
