The information about your graphics card purchase is sent to the email address provided. The actual code isn't included in the packaging. This occurred during orders from ncix at least.

I haven't placed the latest order but I received codes for the previous two promotions—the never settle (including Thief) and the BF4 one. When purchasing the GPUs, they provided a low-quality card that displayed a number; you then registered on the AMD site using your email, entered the code, and received the Steam keys via email. If you select an unreleased title, you'll get a confirmation email with the code arriving after the game's release.

It was included with the card. It had a physical scratchy texture. You could use the code from the AMD site to pick your games, and then they would send you the Steam codes for those titles.
