Beyond extra details, improved lighting and shadows would help, but keep in mind your frame rate could decrease.

Generally, I think the gap between medium and high is clearer than the gap between high and ultra. But as the others point out, it varies by game.

Relies on the game Bit-tech and various platforms perform screenshot comparisons across different graphics levels. Personally, I notice minimal variation between high and ultra settings.

I mostly enjoy fast-paced shooters such as Cod, Be and Medal of Honor. Does that affect my performance? Also, I prefer skiing at medium to high to ultra settings, avoiding low levels.
