What is this card?

Are those five pins? I assumed there were just four, but the third one seems to be a fan connector too... both are correct.
Is there any identifying details on the opposite side of this card? Just a quick note—this isn't a reset switch, right? I'm guessing it's not a network card but part of a USB-based EPROM or ISP programming system. Haha I shared that same idea. "It can't be that old, it has USB..." I checked when USB was introduced. I felt a bit emotional.
I developed my skills on a P2 system, had one internal USB, and later added a two-port expansion card plus an AGP Riva 128.
