RTX 4090 paired with 5800X3D graphics, powered by a 1600W PSU and 32GB RAM. Configured with DocP settings on an Asus B550e motherboard. Cooling performs adequately, maintaining temperatures between 60-70°C during operation.

Have you attempted a fresh installation of the GPU driver? Could you tell me which game this is?

I ran DDU to reinstall drivers. It’s not happening in a game—it’s just when I’m on the main screen in SteamVR (home disabled) while outside VR, everything functions perfectly. The issues appear only in VR, typically during head movements. With three stations I’m not experiencing tracking problems; it worked fine even with two base stations and a 2080Ti.

It might be likely that the drivers aren't fully optimized for VR because the GPU is still relatively new.
