I haven't performed upgrades without backups, so it's uncertain. It's safer to back things up first. I've only upgraded from an old Win8.1 laptop to a new Win10 one.

I wouldn't make any changes unless I had everything safely stored in a completely different storage device.

I usually kept backups but didn’t need to use them often. The upgrade process was always smooth.
