always raise the frequency gradually, one step at a time during OC to reach the stable point.
If you accidentally jumped, reset everything immediately.
For GPU tuning, begin by adjusting the GPU proc frequency and power limits first—don’t change the VRAM frequency yet until you’ve maximized the GPU speed and kept it steady.
If you followed these steps, don’t rely on written settings; just test the VRAM frequency now.

Likely no harm, though it won't be a favorable choice.
Power on in safe mode, reinstall your OC, and begin again.

It seems likely there was no harm, though it won't be a favorable outcome.
Power on in safe mode, reinstall your OC, and try again.
As soon as the incident occurred, several artifacts appeared. I believe it was resolved.

Believe me, many of us have done this too, and it shouldn't cause permanent harm. I did it with one of the 970s I own, and everything is still working fine.

Ah... about damage
Keep an eye on the temperatures during OC. You'll only cause harm if the temps get out of control. As long as they stay within the acceptable range, nothing should be affected. GPU overclocking has been happening for a while, starting with the GTX970.
