I'll try to keep it short but this has been frustrating me for awhile, and there's alot of misinformation out there on the subject, and I'd like a solid answer.
Scenario:
Playing fortnite, with no screen cap, on a 60 hz tv, I get 90-140 fps all the time. With some screen tearing issues, but never drop below 90. My GPU will go up to 99% usage under heavy scenarios.
I don't need 90, so if I cap at 60, I experience drops down to the 30's with my GPU never using more than 50%.
How can I cap to 60, without the drops or low GPU usage.

I've tried every combination of vsync, screen cap combinations through rivatuner, nvidia and the native game ini.
You might want to use DDU to uninstall your GPU drivers and reinstall the drivers with the latest drivers found off of Nvidia's support site though I doubt that might change much since the GPU is already showing signs of it's age. People with that GPU have already had to replace it with something concurrent since their GPU's died.
Also what is the make and model of your PSU and it's age? How old is it? Make and model of your TV as well as the resolution you play at?
When I made that post I had been using a Sony Bravia 60hz television.
Since then, I purchased a Sceptre curved C27 144hz gaming monitor.
my psu is a corsair cx 450w.
Since then, I've noticed a the exact same thing, just relative to my monitor. It's hard to explain but I'll keep it short:
with 60 hz monitor, I could run uncapped and have steady, if not juddery, 130-160 fps. With worst drops hitting 95-100 and GPU using 85-100% and never getting above 70º.
Problem was the screen tearing in the middle of my screen. I can lock rivatuner, nvidia or fortnite native to 60 fps, which kills the judder and the screen tearing, but the GPU doesn't work as hard (40-65% & 45-55º) and I get drops below 40 fps, sometimes in the 20's.
Now with the New 144hz monitor, I can run uncapped getting 130-160 fps again with crazy screen tearing. but when I lock it to 144 fps I get drops into the 60's and 70's (still without going above 70º) without the tear.
Obviously for me it's relative, having come from a 60hz display that couldn't have a steady fps, to a 144hz display that looks definitely smoother than the 60hz but I know I have potential to make it better.

Now I went ahead and reformatted, and upgraded from windows 10 home, to Pro.
On a fresh install, no bloatware, all fresh drivers.
I'm frustrated trying to understand why if I set my frames at 60 I get 20-60 fps, 144 I get 75-144 fps, but if I set it to unlimited I get 130-160. I can set my refresh rate to 120hz with any vsync (fast, on, adaptive, half, native game vsync) it doesnt drop below 120 but its not smooth and without judder. but as soon as I cap it at 120 it drops to 60?\
I'm about to go back to console cuz I don't get it. a ps4 with 60 fps looks smoother on a 60hz tv than my pc looks on a 144hz monitor with any combination of settings.
please help.
edit: isn't vsync fast supposed to throw away the extra rendered frames, so if I ran unlimited frames in game, with vsync fast on 120hz should it not throw away the extra 10-40 frames and sync the 120 fps to my hz?
