I'm running an AMD FX-4300 at 4GHz with an EVGA GTX 750ti SC ACX 2.0. I'm using CryEnb 3 on medium settings and increased the godrays slightly. Currently, I get 25-50 FPS and the graphics look great. Can I raise my average to around 60 FPS without changing the settings much? I've set GTA 4 to its lowest profile, and the GPU is already overclocked slightly.

I believe the issue lies with your CPU, as it struggles to handle open world games effectively.

Your processor load is consistently high, hovering around the 80s and 90%.

My computer seems to handle GTA 4 fairly smoothly... though it might be running a bit slower than ideal, and the game still has some issues.

The CPU is working just fine. GTA 4 has been around for about seven years.
