The machine isn't great. Consider improving it before using it. Tomb Raider generally prefers strong graphics cards; this one seems problematic!
I was able to run it on my old laptop during trips when it was new. It had an i5 processor and a 640MHz clock, giving around 30 frames per second on medium settings. I also made some adjustments and used the hotel’s emergency fan aimed at the exhaust to help improve performance.
The term "VRAM" doesn't accurately reflect the chipset's capabilities, especially when it has a 64-bit bus despite only having 10GB of RAM.
I managed to run it on my Core2Quad with Gt130m, but with fewer cores and a much weaker GPU it would be a rough experience. The minimum AMD card listed is an HD 5870, which is quite different from the 5450. HD 5450 vs HD 5870 for more details http://store.steampowered.com/app/203160/
Your main issue is memory, you're likely to reach about 3GB of RAM usage during TR.
