Hey there,
I joined this forum because the details here inspired me to try overclocking my setup.
Here’s what I have:
Specs: be quiet! Pure Power CM L8 730W voeding
Processor: Intel Core i5-4670K (4x 3400 MHz), Haswell, socket 1150
Cooler: Cooler Master Hyper 412S
Memory: Gigabyte GA-Z87MX-D3H (USB 3.0, sound, LAN, SATA600)
RAM: Kingston HyperX 8 GB (DDR3/1600MHz/CL9), 1x8 GB
GPU: MSI N770 TwinFrogr 2GD5/OC (NVIDIA GeForce GTX 770, 2 GB)
Voltage set around 1.22, frequency at 4.1 GHz
Turbo and C-states turned off
Tried uncore and Ring V too, but everything still doesn’t work well.
Occasionally I get stable at 4.1, but after stress tests it freezes at 4.0. Others claim they reach 4.5+ with similar builds.

Am I missing something or is there a problem with my components?

I believe the chip quality is poor, I also have a 4670k. All I did was sync the cores, multiplier and voltage, but it still failed. It runs at 4.3GHz with 1.22v, but I can't reach 4.4GHz until 1.3v.
