I've checked various solutions such as confirming file integrity, reinstalling the game, removing Steam, moving the game to another drive, and even switching Windows from 11 to 10 with a fresh installation. Still no improvement.

My setup details are here, but I've noticed this issue before—glitches kept popping up even after fresh installs, and I wasn't getting any help from Windows updates. I'm looking for a new GPU now.

I encountered the same issue again, but my system crashed completely last night. I checked for customweapon_sv30 in the files, but nothing showed up.
