Identified the issue. The fan was completely impacting the shroud, with visible scratches on one side—initially puzzling, but later realized two blades were damaged. A significant crack allowed blades to bend outward at higher speeds and strike the shroud. Thanks for the link I received! Now I understand how to replace it. Updated: I was too careless and cheap to get a new fan, so I simply superglued it back together. It functions perfectly now.
