The timing is off at 150MHz or there’s an issue keeping P-states on the GPU active despite heavy power usage. It’s unclear how a GPU wouldn’t consume 260W at that frequency. What does the initial GPU-Z screen display during this stress condition?

Enter the UEFI and toggle on ReBAR. A lot of AM4 motherboards had it as a toggle in the top right on any page, otherwise its also 'above 4G decoding'. I doubt its a singular issue here, but something is preventing the GPU from boosting but not drawing full wattage. I would test ReBAR first, but after that, disable PCIe C-states on the 16x slot. I would then next test Windows Power Plans.

the clock displays correctly, the speed matches 149mhz. the vcore voltage is set at 0.65v. your fans are off at 0% and 0rpm, which suggests they might be faulty. check their functionality—consider using msi afterburner to manually adjust them, or watch the fans during boot to see if they start.

If AMD drivers assume you're running it but aren't, a bug might block proper P-state handling. Power Plans also play a role, influencing P-state and C-state control and being altered by updates. Turning off PCIe C-states in UEFI settings is mostly about removing inconsistencies, not the main problem.
