Hey there! The icons you're seeing might be hidden or disabled, not actual ghost icons. Sometimes apps hide them by default, or they can appear differently depending on settings. Try restarting the app or checking its properties to see if the icons are visible.

This indicates a Palemoon shortcut existed prior to its creation, resulting in the "(2)" designation. Files sharing the same extension must have distinct names, so Windows automatically adds a "(2)" prefix. You should be able to change its name.

The taskbar has just one shortcut, and I looked through the folders too—no additional ones found.
