I have an Asus PG278QR monitor and streaming/gaming on the same PC, with some games running smoothly while others appear laggy. This issue also shows up in OBS. I notice no frame drops, but certain titles behave differently depending on settings like V-Sync. I’m unsure what adjustments are needed to resolve this.
For games that support full CPU vsync at 165Hz, it feels as though having no vsync is the norm, because they rely entirely on the CPU, which will cause performance issues. You'll need to either limit the frame rate or boost the encoding priority (the obs setting), but both choices will lead to reduced FPS in the game.
