Still no solution found
I attempted to reseat all components except the CPU and adjusted the fans so that the top two fans and the front three fans are directing cool air. I also discovered I wasn’t running my XMP profile, which meant the RAM speed was 4800 instead of the expected 6000, but it didn’t resolve the problem. I believed it was fixed for a while until about an hour of gaming caused it to reappear. I narrowed it down to the idea that a game could run normally except when the disk spikes to 100 during gameplay. It seems the issue only affects the disk when I’m actively playing. The resource monitor shows nothing unusual, so I’m still puzzled about what’s happening.

Does your Windows installation use the NVME drive page files system? This might lead to frequent writes, particularly during gaming, and could result in the drive being fully utilized. On my setup, I run Windows 11 with an SSD that has no page files, while enabling them on my game drive. Could you check if using page files on your secondary NVME drive would be suitable for your PC?
