I played GTA Online for about a year and a few times faced issues like being kicked, disconnected, or having network problems. My NAT was strict, so I tried several fixes—such as enabling UPnP and DMZ port forwarding on my router and Windows firewall, routing to my public IP. I also switched ISPs and network types without success. Now I understand that GTA creates new inbound rules when I allow both UDP and TCP traffic through NAT type 3 (Open NAT). Anyone have any suggestions on how to stop this or why it happens?

UPnP enables automatic port forwarding. It lets apps on client devices ask for certain ports to be directed to them. If you wish to prevent this, disable UPnP and set it manually.

The new inbound rules are set within the router, not on the computer's firewall. I can provide guidance but not attach a screenshot.

Computer firewall, sorry, no screenshot since I’ve already removed the new rules. In short, GTA will have two rules—one for TCP and another for UDP. I need to adjust them so edge traffic is allowed, but each time I launch the game it creates a new one that blocks edge traffic. Previously I had around 12 rules stacking up on my firewall.

Sorry, I don't know anything about that game, and a quick google search didn't bring up anything about GTA making rules in Windows Firewall. But earlier when I said that was related to UPnP, that was wrong, because I thought you were talking about port forwarding rules being added to your router.
