I own a TUF 3090 from the Ampere launch. After installing Fortnite and adjusting the settings, my screen went dark with no input errors, yet the system kept running. After rebooting, I reinstalled the drivers but the issue persisted when loading Fortnite again. Once another reboot occurred, I received error code D6 on my MSI Q-Code and the VGA light started up. It seems the graphics card might be failing or there could be other problems. Since your setup uses a custom water cooling system, disassembly won’t be easy—consider checking connections, fans, and thermal paste if possible.

Ooft! That doesn't sound right. The D6 error seems linked to the GPU. I believe removing the system is necessary now. Do you have a CPU with an integrated GPU that can boot at least to confirm it's not a CPU, PSU, motherboard or RAM issue? Perform a thorough build to eliminate shorts. Consider switching PCIe slots (unless you're using an iGPU) – it's a last resort but worth trying. The only sure way is to move the 3090 to another setup and see if the problem persists.
