You remove the wall plates, trust the contractor was reasonable and installed the wiring inside conduit, then replace it with fresh wiring. If they didn’t use conduit, you’ll have to take out the existing floor, install new cables, and lay a new floor (or something similar).
If your telephone cables are running within conduit, this makes it easy. Make sure you know where all the cables converge because that’s where your ethernet will also. Pull ethernet through the same conduit as the telephone cables using the telephone cable to guide the runs. Then terminate the ends with keystones and wall plates or ethernet jacks. At the point where all the cables converge you can terminate with jacks and plug directly into a switch or into a patch panel followed by a switch.
Verify if the cable behind the phone jack is a networking cable with eight wires or a standard phone line with four wires. Since your home was built in 2000 and had phone jacks throughout, you likely used CAT5E cables with four wires. You can simply remove the end of the cable, attach a CAT5E connector and plate to the wall on both ends. If only a four-wire phone line was installed, you’ll need to replace the existing wiring.
