Without a GPU it wouldn't work. Otherwise it relies heavily on the specific GPU model.

I own a GeForce 210 and intend to upgrade it to an AMD Radeon 270. I'm wondering if the CPU could become a limiting factor at that display resolution.

It's likely just a media card, not a full gaming PC. Running GTA 5 on it probably won't work.

Which one are you talking about? Since I mentioned upgrading the graphics card, I’m wondering if I can run GTA 5 on HD without modifying the motherboard, CPU, RAM, or the graphics card itself.

It requires at least two quad cores or a Phenom 9850. Your am2 doesn't support the Phenom 9850. You might not need to upgrade your motherboard if you purchase this, but I suggest upgrading anyway since better bundles are available cheaply. If you still want the game, give it a try and be ready for potential issues.
