I don’t mind less input lag or smoother performance—what matters is feeling it in games. Go ahead and use high settings for visuals.

Usually the VSync illusion is just a trick; it becomes obvious only in the source code where admins can easily spot it.

I've got buddies who claim they don't notice screen tearing, yet at their place it's super clear you can see it. It makes me question what they mean when they say they have no screen tearing without VSync and 1000 FPS.

whether it's a placebo or not, I experience it and if it works better than something less smooth for me, I'll skip it. I'm not going all out with 800x600 stretched just to boost FPS; instead, I'm adjusting one setting from high to high to get about ten more frames per second. That's the approach—it's subtle, you notice it only if you're paying attention.

It means it's still active, even though I'm keeping the settings higher for better frames per second. My monitor is a 1080p at 60Hz.

They won't listen even with premium audio setups, so I'm reducing the frame rate by about 10 fps to highlight the difference. (Also, since you're using 4K at 60Hz, that's a key factor.)
