Using the medium setting, a new server appears as 30-40 on my 1050 Ti laptop. When I run command-line commands, the game becomes very poor with scores over 60.

Absolutely, you're not using the Medium preset. Many game options are turned off by default, leaving you in a lower range. That's why the visuals seem subpar—shadows look dated and lighting appears old-school.

Texture adjustments are the sole factors left intact. Effects like post-processing and distant shadows are turned off, which brightens distant foliage and removes grass or clutter details. Atmospheric shaders are also disabled.
