So I've faced this problem since the game launched. My gaming PC runs most titles smoothly at high settings, keeping a steady 60-50fps. (Examples include Skyrim, Far Cry 3, Black Ops 3, CSGO, Tf2, Witcher 3). Fallout 4 sometimes struggles with the 30-40 region on any graphical setting. I'm unsure why, though—I checked both CPU and GPU usage during Fallout 4 and they stayed at maximum load. (It seems to run surprisingly well around 30-40°C.) Maybe it's because my system is built with AMD? I'm not sure. If anyone else has this issue or knows what's going on, please share. (I've also tried different FPS settings, but results are the same.)

My setup:
CPU: AMD FX-6300
Motherboard: ASUS M5A97 R2.0
GPU: Sapphire Radeon R9 380
Power Supply: Corsair CX750M
HDD: WD Caviar Black 650GB
RAM: Crucial DDR3 8GB (2x4) – name not sure

I believe the problem might be with the CPU. I just moved from FX6300 to I7 6700k and Fallout 4 runs much better now. It no longer drops to low 20 frames and the game feels smoother. Have you checked if updating your graphics card drivers helps? Changing all settings to low doesn’t seem to improve your frame rate either. So it seems Fallout 4 is actually more demanding on the CPU because of the older engine.

Absolutely, the 380 is a solid choice. It should handle the demands of Fallout 4 well.

Fallout 4 runs entirely on the CPU with extremely poor optimization. For better speed, ensure you have at least an i5 processor.

Reduce the shadow distance to medium and set Godrays to the lowest level. I noticed some frame rate drops when looking at certain structures, and adjusting those settings helped lower the performance issues.

Fallout 4 is a demanding CPU application, significantly under-optimized. Excellent gameplay, but I’d avoid it on weaker hardware.

Even with a powerful device, performance won't be consistent at 60 frames per second. Significant problems appear in places such as the Corvega/Lexington area and downtown Boston, where 60fps remains unattainable regardless of adjustments.
