I've checked every driver from Win XP to Win 8, both official and unofficial, and they all face the same issue.

This often indicates a problem with the GPU. You might attempt to update it using a different BIOS for the Sapphire 6970 card.

Your description points to a possible low-level hardware issue, such as a faulty GPU or a conflict with other components. Have you tried using the GPU in a different PCIe slot? Whenever I've faced this problem, it was either due to a defective card or Windows misbehaving with interrupt assignments. Sometimes relocating the card helped, and other times updating Windows resolved it. Still, it's unlikely that 10 or 11 would encounter the same software problems as 9x did, so it probably points to a GPU or motherboard fault. But don't eliminate the possibility of Microsoft causing hardware damage—so a fresh installation isn't impossible.
