I'm not sure why your CPU is rising so much higher than expected under load. I'd suggest updating the BIOS (if you haven't already), clearing the CMOS, and reapplying the undervolt settings. It might not fix anything, but it won't hurt. Honestly, going up to 1.3V isn't a big deal after all.

Have you verified your clock speeds during maximum load? There are motherboards known for attempting auto-OC unlocked CPUs. Not sure, but that’s the only thing that comes to mind.

Clock speed is 4.20ghz, which is why it bothers me, and why auto boost technology isn't the issue...

I also noticed another change—my GPU frequency seems to be decreasing... At some point it's around 500.61mhz... Is this typical?

It's typical for the CPU to slow down when idle, especially under load, even though the usual minimum is often around 800 MHz.
