I would increase your CPU voltage a bit. It seems you should aim for around 1.45-1.46 volts at 4.5ghz. If it works fine at stock settings, maybe your voltage isn't high enough for the overclock. Also think about raising your northbridge by 0.05 volts if you haven't adjusted it yet.
Right now keeping the same settings is stable at 4.3.
