Hi, your GPU has been in use for about seven years. You mainly play Ark Survival Evolved, Black Ops 3, and heavily modded Minecraft with high textures. About two years ago you began experiencing BSODs during gameplay, and a colorful rainbow screen appears occasionally at startup. The issue seems to occur only when playing games, not when idle. It’s clear the hardware is aging—thermal paste application may no longer be effective. Consider replacing it soon rather than waiting.

It seems we're facing the end of VRAM technology, and it might be too late to fix it unless we replace the VRAM chips.

It might be low VRAM. Also, that card is great. I own the EVGA 2GB model.
