Is a 1.26 voltage setting on the GTX 780 secure, or could it cause immediate damage? Your PC experienced crashes right after the GPU reached 1.26 with typical temperatures around 60°C.

From what I've found on reliable forums, 1.25V is the safe upper limit (a drop of 0.01V won't harm it).

As far as i've googled from reputable forums, i believe 1.25V is the maximum safe level (0.01V won't harm it).

Thank you for the details and your patience in searching. I was mistaken about how high my voltage actually reached—it was 1.165. I had another question and hope you can help. I suspect my GPU might be faulty because, during a game (Path of Exile) that runs easily on low-end systems, I experienced crashes. It would spike to 1.125, trigger a VRel warning, and then the whole PC would crash. After adjusting the core voltage on afterburner and lowering the clock speed, it now works smoothly at 1.165 without any crashes. But reaching 1.165 still causes a warning. Before making that change, I used to get a VRel warning at 1.125, but that no longer happens. What do you think? Is it possible the GPU is not working properly?

VREL indicates a voltage reliability problem. What PCIe power connectors does your GTX 780 need? (such as 6-pin and 8-pin). If I estimate, then use different cables (not combining 6+8) – 6 and 8 pin and link them to ensure better stability.

It seems I have two PCIe-E connectors attached to the GPU, each going into a separate PSU. The model is a PNY GTX 780.

It seems like you're hitting the maximum for overclocking, if that's the case.

I believe you're hitting the overclock limit (if you're trying to overclock).
Actually, I'm not overclocking and haven't faced this issue with any other game. I know some people with older NVidia cards that don't have this problem. So maybe I should just consider buying a new PSU (which seems to be recommended in the posts I've seen for POE) and if that doesn't help, get a new GPU.

I'm not sure, generally when I increased my overclocking on the 1050Ti it displayed all the colors in the GPU-Z section.

Im unsure, for most part when i overclocked my 1050Ti it showed all colors on GPU-Z tab,
Which PSU you have currently?
cx 500 watt
It says my gpu core clock is at 862.3 mhz if that means anything and for some reason when i apply an underlcock that number doesn't change whereas the memory clock does if i change it.
Edit: what do you think about this? Now, im not getting vrel warnings before crashing now, voltage doesn't go up nor does fps drop, my temps at like 50c and somehow my whole pc crashes if i go into certain groups of monsters on poe and my pc doesn't even slow down before this happens..... I dont get it. Another thing as well, my whole pc crashes when the heaven stress test starts ( i cant even make it to the bridge )
And another odd thing is today i had poe in windowed mode in the main menu and I had gpuz always on top while i was browsing a site and i noticed randomly that my gpu temps started increasing as if i was playing a game. They started to go into the low 60's and before i knew it my whole pc crashed. This has happened multiple times when i have had poe in the background while poe is at the main menu. I could easily just blame the game but at the same time i crash in heaven stress test ...
