Start with the basics. I'm using Windows 10 and everything is up to date. I've removed and reinstalled various device drivers—controller, video, etc. Now the problem: GTA displays controller buttons but won’t register any input. I’ve tested different XBox controllers (wired and wireless), multiple 360 controllers (wired and wireless), checked the game cache, tried x360ce as an emulator, switched USB ports, and even ran the game in safe mode. All controllers function normally across other games. No software seems to be interfering. Apart from a full Windows reinstall, I’ve done everything possible. This is definitely not due to anything on my end. If this doesn’t help, a fresh Windows install might be necessary.

There aren’t any in-game controls that are disabled by default; the issue likely comes from a setting or toggle you haven’t activated. Check the menu for hidden options and ensure they’re enabled.

I believed they might have included controller toggles, but it appears they didn't.

Does it accept any input or is it just like you didn't even connect the controller?

The controller inputs are displayed but the screen flashes to keyboard inputs instead. When using M/KB it functions perfectly, though trying a controller doesn’t work. Pressing the Guide button causes the screen to flash with each press. A coworker suggested checking the save files, which I plan to try tonight. If that fails, I’ll learn how to use M/KB controls. I really prefer a controller in some games.
