I was using a Core 2 Quad Q9650 with a XFX R7 240 4GB for gaming, and recently upgraded to a Core i5 3450. When I started using the PC, the display became distorted and strange. I've taken snapshots of the issue. After unplugging and plugging in the graphics card again, the problem persisted. If I remove the card's driver when the issue occurs and then restart the PC, it returns to normal, but after a short delay it refreshes and then becomes abnormal again.

Have you tried using a different screen or cable (if available)?
Your CPU includes built-in graphics. Are there any graphics ports on your motherboard you could use instead?

I tested all three HDMI connections on my TV and the PC, the PS4 functions properly with them. After removing the graphics card, I connected via the built-in Display Port to my PC, and it also worked well.

Thanks for the help, I resolved the problem by switching the Pixel format from YCbCr 4:2:2 to Full RGB.
