During gameplay, my computer experiences freezing issues.

My System Details:
Processor: AMD FX-8350 with Eight Cores
Graphics Card: NVIDIA GeForce GTX 970
Motherboard: Gigabyte (970A-D3P)
Storage: Seagate ST1000DM003 and ST4000DM004 Hard Drives
Memory: 2 x 4GB Kingston ValueSelect 99P5471-013.A00LF at 1338MHz (9-9-9-24 timings)
Power Supply Unit: Seasonic M12II 750W EVO Edition

What are the CPU and GPU temperatures and utilization levels while playing, along with the status of disk and RAM usage?
