I have a GTX 1070 Strix and a Ryzen 5 1600 running at 3.8GHz. I recently played Fallout 4 with maximum settings at 1080p and got around 27 frames per second. When I increased the settings further, performance dropped significantly and the game became choppy, jumping unpredictably. It’s hard to play now. I don’t know what’s causing this or how to fix it. All my drivers are current.

The check vsync configuration isn't automatically set to half. You can disable it if needed.

Vsync is disabled, so it shouldn't cause that behavior. The FPS drops from 17 to 33 just by moving around or not firing anything.
