This situation is quite tricky, but I'll try to clarify it. Fallout 4 works properly only when actively running. If you leave it idle or even keep it open on the home screen, your computer's performance will gradually decline. The issue typically becomes noticeable after about five minutes of inactivity. Once you close the game, the PC often remains too slow to use, requiring a full restart to resolve. This problem has resolved itself a couple of times, but not consistently. For instance, if I leave my PC running with Fallout 4 open while I'm away, the frame rate might drop from around 60 fps to as low as 13 fps. I initially suspected a memory leak, but performance issues also occur when playing, which contradicts that idea. It's confusing, so please feel free to ask more questions. Thanks!

It might be a memory leak that activates only during idle periods. Use MSI Afterburner to monitor the graphs and observe the behavior. Allow some time before checking further. Would you like to share any screenshots of the graphs?
