I attempted to overclock my R5 1600 to 4GHz, turned on A-XMP, adjusted the voltage to 1.4V, set the CPU frequency to 4000 with an MSI motherboard, and nothing else. After booting into Windows, my clock speed is stuck at 1.36GHz, while CPU-Z reports a core speed of 1549.63MHz. I’m confused about what happened.

It seems related to the motherboard. If you adjust the voltage above 1.3V, the frequency locks at 1.55GHz automatically. This issue appears with certain MSI and Gigabyte models. A BIOS update might resolve it.
