I understand it's an older title, but for those eager to play without the hassle, create a commandline.txt file in the same directory as GTAIV.exe. Include this line: (fullspecaudio for better audio) This setup is a solid failsafe and should deliver around 50fps on a mid-range rig with medium settings. On my laptop I see 45-90fps at 1080p.
The real impact in-game depends on what you're doing, but the commands generally work as intended. They don't usually clash with graphical settings unless you're adjusting display options separately.
Giving all your GPU's memory to the game ensures better performance, but it may indicate a poor port quality.
