A fresh install is always better, since it's a fresh start, as any windows install tends to get clunky with time. However, I've been upgrading versions of windows ever since 2017, because I had a lot of things installed and it simply wasn't worth the hassle. I have had no problem and everything ran fine. So, it's up to you. If you have a lot of things and it would be a ton of work to reinstall and reconfigure everything, you can upgrade, or if you have a few spare hours, a fresh install would be the way to go.

Updates are installed smoothly, with many people doing it regularly. I perform new installations on schedule due to how I use my device and the locations I visit, as well as the files I download. However, regarding your concern... standard Windows updates will still be applied during a fresh install.
