So here I am once more. I was just trying to focus on my own stuff when a wraith showed up—not from the Niflheimian garrison (near the power place with the tomb you can enter). I started attacking it, but eventually its health bar vanished and when I hit it, it says 'Geralt hit Wraith for 0' and the pain just won't stop. I’m using a silver sword and yrden, but igni doesn’t seem to help. What the hell?

After more than five minutes of pointless criticism, the place became completely empty. It didn’t end up there, it just vanished forever. :mellow:

I believe this occurred to me. It seems to be the White Orchard graveyard, right? Load a previous save, lower the difficulty slightly, use Specter Oil, cast Yrden, and two-shot it—perhaps the extra damage enemies take will be recorded. Or, consider applying a burning/bleeding runestone to your sword; the leftover damage might work. Edit: I was mistaken. The creature you face is the wraith from the Viper Swords scavenger hunt (inside the small church). Don't assume you can defeat it until then.

It was a glitch. It happened to me. Just get away from it. Come back later or save your progress and restart the game. You can wipe it out then.
