I'm facing the same problem again. The only workaround I've tried is switching out the video output cable.

You can simply disconnect and reconnect the HDMI cable for easier use. This should make the problem much less bothersome. I’m still looking for a lasting solution.

The hp monitor has a resolution of 1080p and runs at 60hz. It connects through HDMI. I’m not sure what model it is, but when I turn it off, the screen remains illuminated.

Checked the newest driver updates for your GPU on the AMD website. Your Windows resolution settings seem fine.

I have the newest drivers from Radeon Crimson. I exit normally (in-game) and using the alt tab, it works the same for both. Yes, everything I run is at 1080p (consistent with my Windows settings). Enable borderless windowed mode via the -noborder setting in Steam's advanced launch options. The Source engine can be quite demanding on certain features, with fullscreen being a notable example.
