This is extremely non-descriptive but based on my vague idea it may be a dead drive. you have not listed: 1. specs of the computer 2. what you have done 3. a specific description of the symptoms Do that and maybe someone can give you an intelligent response.

specs: processor - Intel Xeon Chip X5550 running at 2.66 GHz, 95W, 8MB cache, 6.40GT/s QPI, DDR3 1333MHz, overclocked, turbo gpu rx6600 (used a 6-8 pin adapter), ram - 12GB (3x4GB) ECC DDR3-1333 without buffering, issues mainly when powering on and entering f9 boot menu, boot from USB shows "error occurred while accessing the disk" I've reset the BIOS with a CMOS battery, tried two different USB drives and got the same result, no other devices detected except keyboard and USB port remains plugged in.

on bios you see 125gb as solid usb and 512gb as t-force 512gb. both devices are listed as your usb for windows flash, which is under the hard disk section. this area also displays a default values option with a sata defaults tab available. you're currently running a dps self test on your ssd.
