I've been playing this game for almost two years on my PC. It's consistently been a problem, but now I'm beginning to doubt it. On other titles like GTA 5 and Witcher 3, I can maintain around 60 FPS with high settings, and Project Cars works smoothly at very high levels too. However, in Black Flag I only manage about 30 FPS unless I focus on the sky. There might be something specific to this game that caps performance below that, or perhaps my settings or approach aren't optimal. It's a really tough (and not very optimized) game. You'll need to adjust the settings a lot to make it playable in FPS mode, unless your graphics card is powerful enough.

@ Altecice mentioned this too; also consider turning off PhysX entirely. From what I've seen, the PhysX setup in Black Flag often leads to poor performance and stuttering.

I’m using the Nvidia GeForce Experience settings you recommended. I’ve adjusted them slightly myself through GeForce. The game runs fine at around 30fps, which is acceptable. I was puzzled by why some newer games with higher settings gave smoother frames, while this one only hits 30. I’ll test it without PhysX enabled. As mentioned earlier, I stick to the optimized GeForce settings with minor tweaks.

PhysX should be turned off to see if it improves your experience. GeForce Experience usually assumes settings should optimize performance, but they don’t always match real results. It’s better to spend extra time adjusting each setting and checking online guides for tips.

I managed to achieve over 60 frames per second on ACIV, though... it seems I had to turn off all anti-aliasing entirely. A small trade-off for the smooth visuals.

Just for future reference for Unity you will need to do the same but DAMN its a great game once you get the FPS sorted.
