It depends on the update: a newer game usually requires a full redownload, while a smaller patch may simply add content without replacing the entire game.

It will be an optional feature. The original game remains intact, though certain files might be updated. EDIT: In case of a major update, a significant portion of the game could be replaced with new content. However, for expansions, it's unlikely to re-download the original game files.

Which MMORPG are you talking about? For instance, World of Warcraft often releases expansions that also update textures and models, making it clear when changes were made during events like Cataclysm and Warlords of Draenor.
