the original dooms, wolfenstiens, and similar titles don't rely on a y-axis; you simply aim in the general direction and fire. you might hit a few heads occasionally if the enemy's head is aligned, but i don't think it adds extra damage.

In games such as Quake, the headshot system became available almost immediately upon the transition to 3D graphics.

The only thing that comes to mind is robot roller derby with disco dodgeball, where shooter-style mechanics let robots play dodgeball, and getting hit means a single shot.

Yeah I don’t think any modern game will have that. Maybe some will have an option to disable it though. But when I looked, I couldn’t find one.
