I've overclocked my CPU to 4.5ghz stable at 1.37v using the Hyper 212x. It's been almost a year now, and this performance might seem unusual. On OCCT with LLC it reaches up to 1.41v. During stressing it mostly stays around 1.4v, with temperatures reaching as high as 86°C but averaging in the 70s. I'm confident it's safe since it's been functioning well for a year, but I'm curious about the long-term impact. I plan to upgrade both the CPU and cooler in about two to three years.

I believe you're quite capable.
1.4 is the maximum you can reach.
It's the high vcore that harms the processor and raises temperatures.
When it gets too hot, the processor will slow down or shut down if it senses a critical temperature.
Around 100c.

I didn't note that I also had speedstep turned on and ran it on a balanced power plan, which prevents it from running continuously.

Yeah I think I should be able to reach 4.6ghz but it seems like I might have a problem. My friend managed 4.5ghz at 1.32v on his 6600k. Probably just luck.
A max temperature of 86c shouldn't be too much of a concern? I noticed the MSI Core Frozr L can cut my temps by about 10c. Also, it was fluctuating between 1.4 and 1.41, that should be fine?
This is only on OCCT. Usually my vcore will run lower than that. Right now, while I'm typing, I'm idling at 800mhz with 0.896v. Running something like the Heaven benchmark shows all cores are around 4.5ghz at 1.376v.

Higher temperatures and voltages occur during a stress test. Your 70c. performance remains solid under regular load. Using speedstep reduces CPU strain when it's not necessary. You won't notice any difference between 4.5 and 4.6. It provides a pleasant improvement over the standard. Don't worry.
