What kind of screen are you using? Typically, 4k monitors run at 30hz unless you choose a different setting and need a display cable. If your monitor can't handle 60hz in 4k or you're using HDMI, that might be the issue.

VSync remains off but becomes more problematic when turned on. I'm working with a Samsung U28D590D 28-inch panel at 60Hz, using DisplayPort 1.2.

pressing "W" triggers a slight delay, then it moves shortly after. Turning the mouse still causes a minor movement even after stopping.

It seems like input lag is typical when using VSync, but since you mentioned it's turned off, the issue might be with the game itself. I believe it could simply be how the game is designed.
